Does the preservation/limited delamination face and neck lift work as well as the traditional deep plane facelift?

Short answer
Current research shows that both SMAS-based (including preservation/limited delamination) and deep plane facelifts produce meaningful cosmetic improvements with similar safety profiles. Head-to-head comparison data are limited, but some evidence suggests deep plane techniques may offer a modest edge in midface rejuvenation. Neither approach has been proven clearly superior overall.
What the research shows
A large systematic review and meta-analysis of 47 studies covering 10,766 patients found that both SMAS-layer techniques and deep plane facelifts produce significant improvements in appearance and carry similar safety records. Hematoma rates were 3% for deep plane and 2% for SMAS techniques. Nerve injuries were uncommon in both groups, and permanent nerve damage was rare. Only one study in that review directly compared the two, and it found a slight advantage for the deep plane approach in rejuvenating the midface (cheek and nasolabial area). [1]
A separate meta-analysis comparing deep SMAS lift to the MACS (Minimal Access Cranial Suspension) lift, another SMAS-level technique, found comparable aesthetic results and complication rates between those two approaches as well. [7]
A cohort study of 325 patients who underwent a High-SMAS facelift (a deep-layer SMAS technique) reported significant improvements in cheek fullness, jawline definition, and neck contour at one year, with no major complications. [3]
What this means for you
In practical terms, both preservation/limited delamination approaches and traditional deep plane facelifts are considered safe and effective by the current evidence. The deep plane technique may provide somewhat better midface lifting, but this finding comes from very limited direct comparison data. [1] Many surgeons choose the technique based on their training, your specific anatomy, and the areas most important to you. A surgeon experienced in the technique they perform will generally get better results than one using an unfamiliar approach.
Important caveats
The evidence has real limitations. Most studies are not randomized controlled trials that directly pit one technique against another. Outcome measures vary widely between studies, making comparisons difficult.
